This patient was concerned with her eyes. She wanted to preserve her ethnicity but wanted to look younger. She noticed that her lower eyelids had become bulging over the years and that her upper lid skin was beginning to droop. On exam the patient demonstrated excess lower eyelid fat bulge, as well as volume loss in the cheek below the eyelid. Fat grafting was recommended as a related procedure to rejuvenate the lid / cheek junction. This area is called the tear trough and is often an area of aging after age 30. Both the fat grafting and the eyelid surgery was performed in an office setting with local anesthesia alone.
